Schedule Shift: 09:00 AM - 05:00 PM, with a focus on flexibility. Job Description We are seeking an experienced MDS Coordinator for our Skilled Nursing unit. This position involves conducting comprehensive MDS assessments for geriatric patients. The ideal candidate will demonstrate a strong commitment to patient care, ensuring high-quality assessments that adhere to regulatory guidelines. Responsibilities Conducting MDS assessments for geriatric patients. Collaborating with interdisciplinary teams to develop and implement care plans. Meeting regulatory compliance for documentation and patient records. Monitoring and updating patient care assessments as needed. Required Experience / Certifications / Licensure Must have experience as an MDS Coordinator. 2 years of experience in MDS assessments preferred. Current RN license required.
