Experience: Minimum 1 full year of adult home health experience or
1 full year of hospice experience License: Must have license in
hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare
HomeBase and OASIS Description: Provides, coordinates, and directs
the provision of home nursing care according to physician's orders.
Responsibilities are based on agency policies and procedures
through the competent application of the nursing process.
Responsible to the Clinical Manager. Directly supervises and
evaluates Home Health Aides and Licensed Practical Nurses/Licensed
Vocational Nurses. Home visits are required, with periodic office
meetings. Reliable transportation, a current driver's license, and
agency-required liability insurance are necessary. Average patient
load is between 5 to 10 patients per day. Each assigned patient
equates to roughly 6 to 8 points, with a weekly productivity
expectation of 30 points. Types of cases include start of care,
wound care, and follow-up visits. Traveler must be willing to
travel up to 75 miles from the assigned care area. Mileage will be
reimbursed at the IRS standard rate. Documentation must be
completed within 24 hours of the patient appointment. Delayed and
incomplete documentation may impact billing and follow-up patient
care. Candidate must accept every patient assigned regardless of
distance from previous appointments. No guaranteed hours are
provided with this contract. Clinicians may have the opportunity to
float to another clinic if patient census is low. 5 Benefits of
