Minimum Requirements Bachelor of Science in Physical Therapy or Doctoral degree in Physical Therapy from an American Physical Therapy Association approved program - Required. Current State License as a Physical Therapist in North Carolina - Required. Proof of current CPR certification - Required. Current driver’s license, reliable transportation and insurance - Required. 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required. Previous home care experience - Preferred. Ability to perform clinical assessments, prepare and revise Plans of Care, document progress notes, and follow physician and interdisciplinary team recommendations - Required. Ability to order and adapt durable medical equipment and instruct patients and families in safe use of assistive devices - Required. Ability to perform physical demands of the role including lifting, bending, standing and prolonged walking - Required. Ability to travel locally and work a flexible schedule - Required.
