Facility - - - - - And much more---Position Summary:Under the
supervision of the Administrator and according to established
policies and procedures, performs a variety of duties associated
with facility marketing, referral prospecting, clinical evaluation,
admission coordination and processing. -Work is of a confidential
nature. -Performs other work of the department as
requiredCoordinates Admissions from hospitals, other nursing homes,
and various referral souces.Provides screening for potential
residents, including clinical assessment for
appropriateness.Verifies resident eligibility for assistance by
federal, state, community and veteran agencies.Provides information
regarding types of assistance available.Maintains knowledge and
utilizes community resources available to resident. (I.e.
referrals)Notifies potential residents and/or resident
representative Discharge Planner of approved admission.Informs
residents/residents representative or Discharge Planner of
requirement for admission (i.e. PASARR Screening, History and
Physical, Physician's orders, FL-2Effectively communicates
residents status to nursing to allow nursing to prepare for
admission (i.e. resident status,/condition, equipment,
needed).Orients residents and residents representative to facility
policies, routines, advance directives, and financial
responsibilities. Prepares and ensure signature on related
paperwork and documents.2. Communication:Maintains communication
with resident and resident representativeEstablishes and maintains
cooperative and collaborative relationships with outside resource
contacts, so that referral efforts are facilitated and
maximized.Acts as a liaison between resident and resident
representative and facility.Follow progress of resident t aid in
assisting with discharge and transfer.Assist with arranging for
transfer of residents within the facility or to other facilities
when necessary.a. Effectively assist with coordinating the detail
arrangements of transfer.b. Accurately interprets, communicates and
applies the rules and regulations governing eligibility for
assistance.c. Seeks out opportunities to be of assistance in
facilitating the transfer process.d. Maintains knowledge and
utilizes community resources.3. Participates in Quality Assurance
and UR:Effectively assist with data collection, analysis and idea
generation pertinent to customer satisfaction, quality assurance
and quality improvement programs.Provides back-up for other team
members functions in the absence of that employee.4. Performs
miscellaneous duties as needed:a. Participates in other facility
support activities requiring stand-by and re-call at times.b. May
perform occasional weekend assessment or admissions as necessary.c.

Qualifications:Minimum EducationCompletion of a nursing education
program and licensed as a nurse in the state of North Carolina. One
to two years of previous experience in a long term care or related
area required. Knowledge of federal and state programs required.
Experience in interpreting and applying federal and state programs
